Medical practice management plays a big role in preventing such errors However , prevention efforts only be done if the management makes sure that physicians , nurses and other healthcare professionals communicate such incidents . Medical practice management must built and develop a non-punitive environment in to encourage the disclosure of such incidents . The management must promote the reporting of medical error incidents in to formulate a program that would prevent or at least reduce medical errors in the future . These implies that the management must develop a clear guidelines of when , where , what and to whom to report if such incident arise

It is also the ethical duty of the attending physician to admit these mistakes to their patient and disclose the facts necessary to ensure understanding of what had occurred .
