Thus , it would be better if the nurse practitioner would be able to start doing health promotion and disease prevention activities into each routine patient visits through proper health education (Lofton , 2008 ) which is a role of nurse practitioners on primary care . The first step that the nurse practitioner should do is to mingle and know the client . This way , the nurse would be able to understand and know the patient better , together with his /her culture and beliefs . After assessing the client 's health status , the nurse must identify with the client the factors which hinders him from being healthy . The next thing that the nurse can do is to conduct health teachings and encourage these patients by coaching them to further empower the patients to reach their maximum health potential (Hayes Kalmakis , 2007 . Coaching can inspire and motivate these patients to change their lifestyle for a better health
