1 ) Information disclosure

- give accurate and honest information regarding oneself and all necessary information

- never falsify any documents or information

2 ) Choice of providers and plans

- give honest information regarding one 's health status

- never falsify information to get higher benefits

3 ) Access to emergency services

- must follow instructions stated on the plan regarding access of emergency services

- abide by the rules of the hospital regarding the use of emergency services

p 4 ) Participation in treatment decisions

- participate during discussions regarding one 's health status

- involve and inform family members about one 's health status so they can proactively decide on their own , in case the patient is not available

5 ) Respect and non-discrimination

- Also be respectful of whoever health professional caters to him

- not to be selective of who will be of service to him

6 ) Confidentiality of health information

- to inform the health provider /doctor any incorrect or inaccurate information stated on the medical record

- to inform the health provider /doctor if changes have to be made

7 ) Complaints and appeals

- be reasonable in filing for complaints and appeals

- fully review agreements stated on the plan before filing for a complaint or appeal

All of these rights are sanctioned under the Bipartisan Patient Protection Act (Senate bill S .1052
