The most current violations will cost Midwest Racking 418 ,200 . Ormet Primary Aluminum Corp . has had 82 OSHA inspections that date back to 1974 . These inspections have resulted in 315 citations . In 1999 they were fined 8 ,500 for five different violations , including the location of stored materials and the riser levels between steps . In June , 2004 , they were fined 104 ,000 for 17 serious violations and one willful violation . The willful violation was for the failure to install guarding to protect workers from a sprocket wheel and chain . The 17 serious violations included trip and fall hazards , energy control deficiencies , electrical hazards and protective equipment deficiencies The protective equipment deficiencies are a hazard shared by both Ormet and Midwest Racking .
