In the first meeting with Janet a fitness-screening questionnaire (Appendix - I ) covering all areas related to her past and present health condition was handed to her . The questionnaire was set on an easy "yes-no-unknown " format and she completed the same in that session itself . Upon checking her feedback a summary report was prepared like below
