The patient is a 5 y /o female suffering from recurrent metastatic neuroblastoma and is admitted for her third course of IC fenretinide , to be given by continuous infusion over three days she is currently on her second day . She was originally diagnosed last July 31 , 2007

Patient problem (s ) identified ?poor oral intake ? decreased voiding

The patient 's appetite is decreased hence , her oral intake is poor both in food and in fluids . This causes a decrease

in her urine output or voiding , which in turn can be the reason why she is constipated and have not passed stool . She is negative for stool ever since admission

Not swollen , not tender , not hard , tissue surrounding area was soft gauze was dry

PIV -

placed for daily labs per protocol

The site is not swollen , it is not tender to touch , it is not cold there is neither redness nor inflammation Abnormal lab values -

Triglycerides 196 (normal 150

Rerspiratory rate : 20

Heart rate : 115

Temperature : 98 .6

Blood pressure : 90 /60
