The IT application I am dealing with in this is Electronic Health Record (EHR
An EHR is a longitudinal record in an electronic format of the patient 's health and other personal information that may be recorded during one or more interactions with the patient . It may include several types of information including demographic information , personal information familial information , past history , chief complaints , HPI , drug history social history , vital signs , progress notes , immunization history laboratory and X-ray reports , etc . The EHR system is basically created to make the work easier for the healthcare professional , and the health organization , and to be more beneficial to the patient . It would not only be a mere record-making system , but would also have some amount of intelligence in that it would help provide decision-making support for the healthcare professional . It would work providing relevant evidence-based information and in this way improve the clinical outcome and the quality standards . An EHR may be chosen to be functional in a particular organization , or across several organizations , region or even an entire nation . They can be customized to suit a particular department , enterprise or purpose . EHR were first utilized in the 1960 's , when several hospitals began to utilize EHRs to better manage the patients (NIH NICRR , 2006 When a hospital has a complex structure , such as several clinical units , specialty units , inpatient facilities , diagnostic centers laboratories , radiology units , etc , then an EHR could help in proper documentation , and transfer of the patient information . Besides , an organization that would be doing research would also gain as patient information could be put to potential benefits .
