As a system it may be unable it does not focus on preventive aspects , and may either under-treat or over-treat the patients . The clinical responsibility by the managed care organization is also poor , as healthcare services are provided by a healthcare consisting of a physician and several other healthcare professionals . More patient work is allocated to other healthcare professionals rather than the physician . Managed care actually began in the early part of the 20th century . Several companies began to operate medical services or got into contracts with healthcare organizations to provide healthcare services to their employees Employee associations were formed to ensure that the workers were able to obtain healthcare services from these healthcare organizations or physicians . These healthcare providers agreed to provide medical care for a prepaid amount . The employers and the employees benefited hugely from this scheme as medical care was effectively 20 to 40 less in terms of cost (Fairfield , 1997 . The Government slowly became interested in such schemes and slowly began to develop a system by which the people could access healthcare services from organizations setup in a corporate fashion
