Health care providers in emergency departments (EDs ) are continually faced with a challenging array of ethical dilemmas , which are generally multifactorial in nature . Several such factors that create ethical concerns in the ED include the acuity of injuries that require immediate care without sufficient time for consultation and deliberation difficult balance between patient care and resource needs rapid decision making , and the inability

on the part of the patient to participate in the medical decision-making process because of altered mental status and /or the lack of prior relationship between physician and patient . To a large extent , such ethical dilemmas involve a fine balance between physicians ' beliefs regarding the best interests of the patient and other patient-oriented issues such as autonomy and privacy In addition , such ethical challenges are significantly magnified during large-scale disasters such as attacks of violence and terrorism , which can result in mass casualties . In such situations , priorities are likely to be shifted from the needs of individual patients to that of the community and society as a whole (Marer , et .al , 2004
